Soybean oil 200 g, egg lecithin 12 g, glycerol 25 g. Energy: 2,000 kCal
Indications/Uses
As source of energy & essential fatty acids.
Dosage/Direction for Use
IV infusion Adult Not exceeding 2 g lipids/kg/24 hr. Max: 3 g lipids/kg/24 hr. Infant & neonate Recommended dose: 0.5-4 g lipids/kg/24 hr. Initially 0.5-1 g/kg/24 hr then increase by 0.5-1 g/kg/24 hr up to 2 g/kg/24 hr. Very low birth wt infant & small for gestational age infant Recommended initial dose: 0.5 g lipids/kg/24 hr, then increase to 4 g/kg/24 hr in relation to the infant's ability to eliminate fat & w/ close monitoring of serum triglyceride conc, liver tests & oxygen saturation.
Contraindications
Severe hemorrhagic diatheses, imbalanced DM in unstable metabolism, allergy in chicken protein, recent cardiac infarction, stroke, embolism, coma of unexplained cause, severe liver damage & shock. 1st trimester of pregnancy.
Special Precautions
Uncompensated DM, pancreatitis, impaired liver function, hypothyroidism (if hypertriglyceridemic) & sepsis. Perform daily lipid clearance evaluation. Renal insufficiency. Pregnancy. Neonates & premature w/ hyperbilirubine­mia, in cases w/ suspected pulmonary HTN.
Adverse Reactions
Acute reactions: Allergic reaction, hyperlipidemia, nausea, vomiting, cyanosis, dyspnea, sweating, headache, chest & back pain, flushing, sleepiness, hyperthermia, dizziness, thrombocytopenia (rarely in neonates), hypercoagulability, slight pressure over the eyes & irritation at the site of infusion. Chronic (delayed) reactions: Hepatomegaly, jaundice due to central lobular cholestasis, splenomegaly, thrombocytopenia, leucopenia, transient increase in LFTs & overloading syndrome.
